All athletes need to update their Home Campus profiles now!

1) All athletes must have a current physical to participate.

2) All athletes must have a profile on: www.homecampus.com 

 

3) All athletes must complete ImPact testing

 

4) Athletic Clearance Profile and ImPact/Concussion testing directions can be found under the 'Athletics Info' tab

You must use the HBUHSD Physical form which can be found here. Please note the first page needs to be completed and signed by a parent/guardian and the doctor will fill out, sign and date the second page.
